I have read the Constitution/By Laws, Ethics Statement and Professional Practices Policy of the Federation of Christian Ministries and agree to abide by the conditions specified in these documents:

Dues payments are for 2011 - January 1st. through December 31st 2011
To remain a member in good standing all dues and annual reports must be submitted no later than December 15, 2010

Non-Certified/Commissioned member dues:	$40 ($50 after 12/15/10)
Certified/Commissioned member dues and certification renewal fee:	$120 ($150 after 12/15/10
Professional Endorsed Membership dues & fees:	$195 ($210 after 12/15/10

International Council of Community Churches (ICCC)
Membership

Any certified FCM member is eligible to become an Affiliated Minister with the
International Council of Community Churches (ICCC).

THE 2011 CAMPAIGN ENDED DECEMBER 15, 2010.
Next application date is October 2011.
